. Apply resistance just above the wrist forcing the arm back down to. This mechanism is similar to the McMurray test for a torn meniscus in the knee. Speeds test was positive in 40 shoulders.

However it is positive with a various number of other shoulder pathologies. A common variation of the test is described as the patient moving the shoulder into external rotation while simultaneously moving the forearm into the supine position too against resistance. OBrians Test In the OBrians test the patient is instructed to first flex the shoulder at 90 and then to adduct it of 10.

A positive test is considered if there is significantly greater pain in the bicipital groove with the arm supinated. Start with the patients arm in 90 degrees of shoulder flexion. A click associated with pain makes the test positive.

Have the patient resist a movement into extension a first time supinated and a second time with the arm pronated. He concluded that the Speeds test is a non-specific but sensitive test for macroscopic bicepslabral pathology. Patient is seated Patient completely extends their elbow then supinates their arm Therapist stabilizes at the shoulder Patient attempts to flex the elbow while the therapist holds clients forearm and applies resistance.

To perform the Speeds Test position the patient in either standing or sitting with their elbow full extended and forearm fully supinated. Prospective blinded study of consecutive patients with a wide spectrum of shoulder conditions. - Crank Test - performed with the patient lying and elevating the shoulder with the elbow flexed at 90 degrees.

For superior labral SLAP tears the summary sensitivity and specificity of the Speed test was 32 and 61 respectively. The same used for biceps tendinopahty due to the insertion of the biceps tendon on the superior aspect of the glenoid labrum. It is a complex movement and the patient is encouraged to perform it without resistance first so as to be able to elicit effective response when resistance is added.

A meta-analysis in 2008 concluded that the diagnostic accuracy of individual tests in the shoulder examination was limited specifically that the Hawkins-Kennedy test and the Speed test have no discriminatory ability to diagnose specific shoulder pathology and that results of studies evaluating other tests were too statistically heterogeneous to make. Bicepslabral pathology was present in 10 of these patients. The Speeds test is useful for assessing biceps tendinopathy as well as labral pathology specifically SLAP lesions.

31 rivi Speeds Test Biceps Test Arm is extended behind and the forearm supinated with elbow. The purpose of this study was to explore and describe reasons for variation in diagnostic accuracy of clinical tests using Yergasons and Speeds tests in predicting biceps tendon pathology and SLAP lesions. An axial load is applied while the arm is rotated internally and externally and circumducted.

Clinical demonstration of Speeds test The patient is standing or seated Arms are flexed forward to 90 arms supinated The patient is asked to bring the arm into further flexion above 90.
